Six-Month Certification Requirement - Page 1

If you were deemed eligible to participate in the Limetree Water Distribution Program (the “Water Program”), then in order to maintain eligibility, you must certify every six months that your eligibility circumstances have not changed. No additional documentation other than this certification is necessary.

You must re-certify within 30 days of the date listed on the Recertification Notice Letter you received. Please note that if you do not provide the six-month certification, your Water Program eligibility may be revoked.

In order to maintain eligibility, please confirm your name, physical address within the Covered Area, and certify under penalty of perjury that the circumstances which made your claim eligible have not changed.

You can provide your six-month certification on this website by continuing below, or by returning the notice mailed to you with your printed name, signature, and date to the Limetree Water Program Administrator by email to info@LimetreeWaterProgram.com, or by mail to the following address:
